ChiroCenter One and Dr. Jongeward is DOT exam certified and a member of the National Registry of Certified Medical Examiners. As of May 21, 2014 DOT driver exams can only be performed by Certified Medical Examiners.

Commercial Drivers are required to have DOT physical examinations for a Commercial Drivers License (CDL) at least every 2 years, and more often in some instances such as high blood pressure, diabetes, and sleep apnea.

The examination includes vision, hearing, blood pressure, lab tests, and a physical examination.

Once all criteria are met, the driver will be issued a Medical Examiner's Certificate and given a copy of the long form.

Preparing for Your Examination

All drivers must come to the examination with a completed drivers section of the Medical Examiners Report Form (the first and second page). Prescription names, treating physician names and address, and any other documentation for current or recent conditions and injuries are an essential part of the review. Incomplete information and forms will result in a delay or denial of the Medical Examiner's Certificate.

If you are currently using a CPAP machine, you must bring proof of usage. You can bring the machine itself if it shows times of use, or you can have your sleep center print a copy that shows usage times.
